In detail

Getting a passport in Utah

Utah does not have a State Department passport agency, so most residents apply for a first passport at an acceptance facility such as a post office, a county clerk's office or a library. For urgent travel the closest agency is in another state, which is why many Utahns use a registered courier such as Allstate Passports & Visas. This page covers 11 Utah cities, explains each option, and shows how to order a Utah birth certificate.

Acceptance facilities in Utah

Acceptance facilities process first passports, passports for children and replacements for lost or stolen passports. Many work by appointment, so plan ahead. The city pages above list the facilities in each area.

Bring Form DS-11, unsigned until the agent asks, proof of U.S. citizenship such as a certified birth certificate, a photo ID with a photocopy, and one passport photo. There are two fees: one to the U.S. Department of State and one to the facility.

The nearest passport agencies

From Salt Lake City and the Wasatch Front, the closest State Department agency is the Colorado Passport Agency in the Denver area, roughly 500 miles by road. From St. George, the Los Angeles Passport Agency is closer. Every agency sees applicants by appointment only, and only if they travel internationally within 14 days or need a foreign visa within 28 days. How to book an agency appointment.

Birth certificates in Utah

The Utah Department of Health and Human Services, Office of Vital Records and Statistics in Salt Lake City, has birth records from 1905 to the present. The state's current application form lists $25 for the search and one certified copy, with expedited processing available for an extra fee. You can order online at vitalrecords.utah.gov, by mail, or in person at a local health department.

Utah birth records are not public until 100 years after the birth. You can order your own certificate, or one for a parent, sibling, spouse, child, grandparent or grandchild, with government-issued ID. Anyone else needs documents proving a relationship or legal need. Details are on the Utah Office of Vital Records and Statistics website.
