Eligibility and Document Requirements

You are eligible to apply for a passport card if your most recent U.S. passport:

  • Is undamaged and can be submitted with your application
  • Was issued when you were age 16 or older
  • Was issued within the last 15 years
  • Was issued in your current name or you can legally document your name change

Step 1

Complete and Submit Form DS-82: Application For A U.S. Passport

Complete Form DS-82: Application for a US Passport Cart (Online Version) . The US Passport Agency has introduced an Online based application form, this Online style application must be used in all expediting cases. Failure to use this Online application may cause delays in processing times. TIP: This Online passport card application has replaced “Hand Written” applications, YOU ARE NOT APPLYING
ONLINE you are only completing the questions by typing them onto the application the US State Department web site ‘Wizard’ will walk you through the application.

When prompted to select type of service, check “Passport Card”only.

The last page of the application is a Print summary page, please scroll to the bottom left corner of this page and CLICK CREATE FORM you will then be able to print (single sided only) and sign (in blue or black ink only) your application.

Note: The application Wizard will ask you to select fees, shipping and processing options. Select any options to complete the form, no payments will be made and options selected will have no effect on the actual service provided by Allstate Passports & Visas. The Wizard is simply designed to assist the general public in calculating fees for traditional mail in applications. A correctly formatted application should have a BAR CODE on the top left corner, and DS-82 printed on the bottom left of the application.

Submit Your Most Recent U.S. Passport

Include your most recent undamaged U.S. Passport

Step 3

Submit Evidence of Name Change, If Applicable

Include a marriage certificate or court order (only if your current name is different from what was recorded in your most recent U.S. Passport).

Step 4

Pay the Applicable U.S. Government Fee

The cost of the passport card is included:

  • Government Fees for applicants of age 16 and older: check or money order payable to “U.S. Department of State” for $90
    -or
  • Government Fees for applicants under 16 years old: check or money order payable to “U.S. Department of State” for $75
Step 5

Provide Two Professional Passport Photos

Your Photographs for passport card renewal Must Be:

  • Identical
  • In color
  • 2 x 2 inches in size
  • Taken within the past 6 months
  • Full face, front view with a plain white or off-white background
  • Taken without eye glasses

Include Two Letters of Authorization

Complete the Letter of Authorization for Allstate Passports & Visas in DUPLICATE.

  • Must list your full legal name
  • Must have your original signature and date

Please sign two Letters of Authorization with original signature (BLUE or BLACK ink ONLY).

Step 7

Include Proof of Travel

24-72 hours expedited passport card services require proof of travel within 14 days.
Proof of travel may be one of the following:

  • A personal statement addressed to “US Passport Agency” detailing your travel plans including method of transportation and planned accommodation
  •  If traveling to Canada or Mexico by land or sea, applicant may submit a hotel reservation or other document detailing the need for a passport card
  • A letter from your employer confirming the need to travel in the immediate future (sample)

Where can I use a passport card?

You can use a passport card when entering the United States at land border crossings and sea ports-of-entry from:

Canada
Mexico
The Caribbean
Bermuda

Real ID: Passport card is also an alternative to a state-issued ID.

Can I use a passport card to fly internationally?

No. The passport card was designed for the specific needs of northern and southern U.S. border communities with residents that cross the border frequently by land. The passport book is the only document approved for international air travel.

Can I use a passport card at Ready Lanes?

Yes. Adults and children with U.S. passport cards can save time at the U.S.-Mexico border by using designated Ready Lanes that read the passport card’s Radio Frequency Identification (RFID) technology.

How to use Ready Lanes with a U.S. passport card:

Navigate to a lane that has the Ready Lane logo
Keep your passport card in hand
Display the passport card to the RFID card readers
Proceed to a U.S. Customs and Border Protection (CBP) officer

How long will my new passport card be valid?

If you are 16 or older your passport card is valid for 10 years.

If you are under 16 your passport card is valid for 5 years.
