Bring Form DS-11 (leave it unsigned until the agent tells you to sign), proof of U.S. citizenship such as a certified birth certificate, a photo ID and a photocopy of it, and one passport photo. You pay the application fee to the U.S. Department of State and an execution fee to the facility.

Birth certificates in New York

Born in New York City (the Bronx, Brooklyn, Manhattan, Queens or Staten Island): the NYC Department of Health and Mental Hygiene issues birth certificates for births since 1910. Each copy costs $15, and online orders through VitalChek, the department's only authorized vendor, add a processing fee. Records from before 1910 are held by the NYC Municipal Archives. Details are on the NYC Health Department birth certificate page.

Born anywhere else in New York State: the New York State Department of Health has birth records from 1881. A copy costs $30 by mail, or $45 plus a vendor fee online or by phone. The local registrar in the town or city of birth can also issue copies. Only the person named, a parent named on the certificate, or others with a New York court order can order. Details are on the New York State Department of Health birth certificate page.

New York passport questions

Is there a passport office in New York?

Yes, two. The New York Passport Agency in Manhattan and the Buffalo Passport Agency both see people by appointment only, for travel within 14 days or a visa needed within 28 days. Everyone else applies at an acceptance facility or renews by mail or online.

Where do I get a birth certificate if I was born in New York City?

From the NYC Department of Health and Mental Hygiene, not the state. It issues certificates for births in the five boroughs since 1910 for $15 each. The New York State Department of Health cannot issue New York City birth certificates.

Can I renew my passport online in New York?

The State Department runs online renewal for the whole country, so New York residents follow the same rules as everyone else. It works for adults renewing a regular passport who are not traveling soon. If your trip is close, book an agency appointment or use a registered courier.

Can I get a passport the same day in Manhattan?

Only with an appointment at the New York Passport Agency, and only if you travel within 14 days or need a visa within 28 days. Same-day issue is not guaranteed and depends on your case.
