Passport acceptance facilities
Acceptance facilities handle first-time passports, passports for children under 16, and replacements for passports that were lost or stolen. Many county clerk offices and post offices in New Jersey take appointments only, and some also take passport photos on site. Pick your town above to see the facilities nearby.
For your appointment, bring Form DS-11 unsigned (you sign it in front of the agent), proof of U.S. citizenship such as a certified birth certificate, a photo ID with a photocopy of the front and back, and one passport photo. The application fee goes to the U.S. Department of State and a separate execution fee goes to the facility.

Birth certificates in New Jersey
The New Jersey Department of Health, Office of Vital Statistics and Registry in Trenton, keeps state birth records from 1918 to the present. Older records, from May 1848 up to about 100 years ago, are held by the New Jersey State Archives. A certified copy from the state office costs $25, and extra copies of the same record ordered at the same time cost $2 each. You can order online or by phone through VitalChek, or by mail.
You can also order from the local registrar in the town where the birth took place. Fees there are set by each town, and a local office is often the quickest choice if you live nearby. Every request needs a copy of your photo ID and proof of your relationship to the person named on the record. Details are on the New Jersey Department of Health ordering and fees page.

Which passport agency is closer to me, New York or Philadelphia?
For North and Central New Jersey, the New York Passport Agency in Manhattan is usually closer. For Camden, Burlington, Atlantic and Cumberland counties, the Philadelphia Passport Agency is usually the shorter trip.
Can I renew my passport online in New Jersey?
Yes, if you qualify. Online renewal is run by the State Department and works the same way in every state. It is meant for adults renewing a regular passport who are not traveling soon. If your trip is close, an agency appointment or a registered courier is the faster route.
