Getting a passport in Arizona

Arizona is home to a State Department passport office, the Western Passport Center in Tucson, and to acceptance facilities in post offices and county offices around the state. Most Arizonans apply for a first passport at a local facility. If you travel soon, you can book an appointment in Tucson or use a registered courier such as Allstate Passports & Visas. A first passport, a child's passport or a replacement for a lost or stolen passport starts at an acceptance facility. Around Phoenix and Tucson, many post offices and county offices offer passport appointments. The city pages above list the facilities near you.

On the day, have Form DS-11 ready but unsigned, a certified birth certificate or other proof of citizenship, a photo ID with a copy of the front and back, and one passport photo. The passport fee goes to the U.S. Department of State, and the facility charges its own execution fee.

The Western Passport Center sees applicants by appointment only, and only if they travel internationally within 14 days or need a foreign visa within 28 days. Birth certificates in Arizona

The Arizona Department of Health Services, Bureau of Vital Records, has birth records from July 1909. A certified birth certificate costs $20. Besides the state office, several county vital records offices can issue certificates for births anywhere in Arizona, and many take walk-ins. You can also order online, with an added processing fee.
