Complete Passport Application Form

Complete Passport Application Form DS-11 online, print single sided. DO NOT sign the form yet.

Take printed and unsigned Passport Application Form together with required documents from the checklist (above) to the nearest Passport Application Acceptance Facility.

At the Passport Application Acceptance Facility

Both parents must appear in person with the minor. A Passport Specialist will verify your identity, review the documents and direct you to sign the Application Form.

Your documents will be placed in a sealed envelope. It is possible that not all documents from the checklist above will be included in a sealed envelope (every Passport Application Acceptance Facility has it's own guidelines). In this case be sure to attach with a paper clip the remainder of documents that were not sealed on top of the sealed envelope.

Let the Passport Specialist know that you will be using a "Hand Carry" expediting service to process your application.

Print out the "Hand Carry" procedure guidelines form and bring with you, in case the Passport Specialist has questions about "Hand Carry" expediting procedure. (more information)

DO NOT mail your sealed envelope to the National Passport Center. Ask the Passport Specialist to return the sealed envelope to you.

Once the sealed envelope is handed to you, please write your child's : First and Last Name, Date of Birth and Date of Departure.

You will send the sealed envelope to Allstate Passports & Visas using a FedEx label we provide.

Ship documents

Mail the sealed envelope to APV office using a prepaid FedEx shipping label provided by us. Retain a copy of shipping receipt for your record.

VERY IMPORTANT: We must receive and submit your sealed documents to the Passport Agency within four business days from the time it was sealed at the Passport Application Acceptance Facility. DO NOT delay sending the sealed envelope to Allstate Passports & Visas. If the sealed documents are not submitted to the Passport Agency within four business days from the seal date on the envelope, they become invalid and will be rejected.
