Bring a completed Form DS-11 (do not sign it until the agent asks you to), proof of U.S. citizenship such as a certified birth certificate, a valid photo ID with a photocopy, and one passport photo. You pay two separate fees: one to the U.S. Department of State and one to the facility.
The Seattle Passport Agency
The Seattle Passport Agency is run by the U.S. Department of State. It only sees applicants with an appointment who travel internationally within 14 days or need a foreign visa within 28 days. Bring proof of your trip, such as a ticket or itinerary. How to book an agency appointment.
Allstate Passports & Visas
We are a private courier registered with the U.S. Department of State, not a government office. Call (800) 672-1015, text (206) 522-5995, or read about our passport services.
Birth certificates in Washington
The Washington State Department of Health, Center for Health Statistics, keeps birth records from July 1, 1907. A certified copy costs $25, plus a processing fee that depends on how you order. You can order online or by phone through VitalChek, the department's only contracted vendor, by mail, or in person at many local health departments. Mail orders can take several weeks, so order early if your trip is close.
Since January 1, 2021, Washington issues certified birth certificates only to the person named, close family members, legal representatives and others who can prove a qualifying relationship. Details are on the Department of Health vital records page.
