You Are Eligible To Use This Service If Your Most Recent U.S. Passport:

  • Is undamaged and can be submitted with your application;
  • Was issued when you were age 16 or older;
  • Was issued within the last 15 years and more than 1 year ago;
  • You can legally document your name change

After placing an order, we will contact you within 1 hour (during normal business hours) at the phone number you provide to review your order for an expedited passport. We will answer any questions you may have, we will then go over the passport name change checklist and provide you with the FedEx inbound shipping label to ship your documents to us for an expedited processing.

By using our service, you secure an expedited processing slot reserved specifically for registered passport couriers, ensuring that your documents are handled within your chosen timeframe. We will then personally hand-deliver your passport application to a regional passport issuing office.

Complete and Submit Form DS-82: Application For A U.S. Passport

Complete Form DS-82: Application for a U.S. Passport (Online Version) . The US Passport Agency has introduced an Online based application form, this Online style application must be used in all expediting cases. Failure to use this Online application may cause delays in processing times. TIP: This Online application has replaced “Hand Written” applications, YOU ARE NOT APPLYING
ONLINE you are only completing the questions by typing them onto the application the US State Department web site ‘Wizard’ will walk you through the application.

The last page of the application is a Print summary page, please scroll to the bottom left corner of this page and CLICK CREATE FORM you will then be able to print (single sided only) and sign (in blue or black ink only) your application.

Note: The application Wizard will ask you to select fees, shipping and processing options. Select any options to complete the form, no payments will be made and options selected will have no effect on the actual service provided by Allstate Passports & Visas. The Wizard is simply designed to assist the general public in calculating fees for traditional mail in applications. A correctly formatted application should have a BAR CODE on the top left corner, and DS-82 printed on the bottom left of the application.

Submit Evidence of change name on passport

Include original or certified marriage certificate, divorce decree or court order (only if your current name is different from what was recorded in your most recent U.S. Passport).

Pay the Applicable U.S. Government Fee

  • Government Fees for applicants of age 16 and older: check or money order payable to “U.S. Department of State” for $190, if applying for a Passport Book or $220, if applying for a us passport name change and Card

Provide Two Professional Passport Photos

Your Photographs Must Be:

  • Identical
  • In color
  • 2 x 2 inches in size
  • Taken within the past 6 months
  • Full face, front view with a plain white or off-white background
  • Taken without eye glasses

Include Two Letters of Authorization

Complete the Letter of Authorization for Allstate Passports & Visas in DUPLICATE.

  • Must list your full legal name
  • Must have your original signature and date

Passport Name Change FAQ

Is name change required due to marriage or divorce?

It is not required but recommended. U.S. citizens who change their names may travel using a U.S. passport that was issued in their prior name, but need proof of the name progression, according to U.S. Customs and Border Protection (e.g. marriage certificate, divorce decree or court documents showing the legal name change).

It is recommended that travelers check with the countries they are traveling to for their entry requirements.

U.S. CBP also recommends calling the Transportation Security Administration to find out their latest requirements about name matching on travel documents.

For more seamless travel, it's a good idea to change the name on your passport.
