Passport Offices and Expedited Passports in Wisconsin
Wisconsin has no State Department passport agency, but most residents are within reach of Chicago or Minneapolis, and the state makes birth certificates easy to get. Most people apply for a first passport at a post office or a county clerk's office. Passport acceptance facilities
You apply in person at an acceptance facility for a first passport, a child's passport, or to replace a lost or stolen one. In Wisconsin many county clerk offices and post offices take passport appointments. Your city page above lists the facilities nearby.
Come with Form DS-11 (sign it only when the agent tells you to), a certified birth certificate or other proof of citizenship, a photo ID and a photocopy, and a passport photo. There is one fee for the U.S. Department of State and a separate one for the facility.

Birth certificates in Wisconsin
Wisconsin has statewide issuance: if you were born in Wisconsin on or after October 1, 1907, any county Register of Deeds office can print your certified birth certificate, whichever county you were born in. In person it often takes about 15 minutes. The fee is the same everywhere, $20 for the first copy and $3 for each extra copy of the same record. Many offices also take mail orders and online orders through VitalChek.
A certified copy is only available to someone with a direct and tangible interest: the person named, immediate family such as a parent, current spouse, sibling, grandparent or child, a legal custodian or guardian, or a representative they authorize in writing. Bring valid ID. Details are on the Milwaukee County Register of Deeds vital records page, which explains the statewide rules.
