Passport acceptance facilities

Applying for a first passport, a child's passport or a replacement for a lost or stolen one means visiting an acceptance facility in person. In West Virginia many post offices and county clerk offices handle passports. Your city page above lists the facilities nearby.

Bring Form DS-11 but leave it unsigned, a certified birth certificate or other proof of citizenship, a photo ID with a copy, and one passport photo. There is a fee for the U.S. Department of State and another for the facility.

The nearest passport agencies

From Charles Town and Martinsburg, the Washington Passport Agency in DC is a little over an hour away, and from Morgantown it is the nearest too. From Huntington, Charleston and Southern West Virginia, every current agency is a long drive. The State Department is building a Cincinnati Passport Agency, which will be closer for the western part of the state once it opens. Birth certificates in West Virginia

The West Virginia Vital Registration Office has birth records from 1917, and a certified copy costs $12. The fastest way to get one is in person: the office at 350 Capitol Street, Room 165, in Charleston offers same-day walk-in service. Mail orders take about one to three weeks, and VitalChek is the only online and phone service the office approves. For births before 1917, check with the county clerk in the county of birth.

Certified copies go to the person named, parents, grandparents, a spouse, children, grandchildren, brothers and sisters, or someone with a legal interest, and you must show ID. Details are on the West Virginia Vital Registration certificate requests page.
